Medical Triage Specialist - LPN
Program: Journey of Hope
Reports to: Manager of Medical Services
Location: Main Campus and or Moon Campus
Hours: Full-time - Exempt – 40 hours per week
MISSION AND NEED: Holy Family Institute (HFI) is a 120-year-old Catholic organization with an impeccable history of serving children and families in the Pittsburgh Region. We are one of Pittsburgh’s largest social service organizations operating 15 programs in three counties. We are an independent organization guided by Catholic Social Teachings and serving people of all faiths and backgrounds.
General Statement of Duties:
This position is responsible for overseeing the medical needs of the shelter and long-term foster care children. The Medical Triage Specialist-LPN would conduct assessments and administer medications to children.
Essential Functions:
- Triages phone calls from staff regarding medical incidents or injuries and responds to assist the minor(s). Minor assessments and interventions are completed on campus, if medical follow up is needed, minor is scheduled for an appointment or taken to the ER.
- Report any changes in minors’ health to medical managers, doctors and nurses.
- Provide education and guidance to staff regarding the minor’s medical condition and needs that this diagnosis requires.
- Monitor medication administration. Ensures that the minor’s receiving medication have received per healthcare provider’s order.
- Ensure that house/cottage have medications and medical supplies needed to care for the minors.
- Transport minors to appointments if necessary.
- Pick up medications as needed.
- Administer medications (per license or certification).
- Keep and maintain detailed records for the minors in care of HFI. The minor’s record must be accurate, timely and complete (this includes portal updates).
- Gather and record data related to HFI medical department, and the services provided to the minors while at HFI.
- Complete internal and external incident reports including child level events (reports to ORR).
- Schedule appointments for minor(s).
- Prepare documentation for intakes and discharges.
- Maintain client confidentiality.
- Attends all mandatory training.
- Practicum Observer through Medication Administration training.
- Work weekends & holidays per schedule.
- Participate in on-call rotation.
- Conducts medical intake assessment & interviews of minors, following timeframe to meet federal and state regulation. This would include taking a health history, vitals and completing eye and ear checks.
- Conducts discharge assessments.
- Assists and promotes acculturation in a culturally sensitive way.
- Models and assists in the implementation of the HART and Trauma-Informed Care.
- Exhibits cultural competency and sensitivity.
- Participates in all required agency and regulatory training within the expected time frame.
- Works collaboratively with staff, children, families, service providers, and others.
- Promptly reports civil or criminal convictions, charges, arrests, investigations, or adjudications that may arise since staff member’s last background investigation. This includes but is not limited to sexual abuse in a prison, jail, holding facility, community confinement facility, juvenile facility, other institution (as defined in 42 U.S.C. 1997), or care provider facility; conviction of engaging or attempting to engage in sexual activity facilitated by force, overt or implied threats of force, or coercion, or if the victim did not consent or was unable to consent or refuse; or who was civilly or administratively adjudicated to have engaged in such activity.
- Completes other duties as assigned by supervisor.
Minimum Qualifications:
- Licensed Practical Nurse (LPN) or Registered Nurse (RN), required.
- One year previous clinical, hospital, school, or nursing experience preferred. Pediatric experience preferred.
- Ability to communicate effectively and document information accurately.
- Ability to deal efficiently and effectively with a wide variety of individuals. Ability to maintain electronic records, medical files and have basic computer skills, knowledge of Microsoft Office applications.
- Ability to fully read, write, and speak in English required. Bilingual: Spanish and English preferred.
- Must have Act 33/34 and FBI clearances and valid PA driver’s license
